Onboarding a Virtual Assistant into Your Optometry Practice: The First 30 Days

Here's the pattern behind nearly every failed virtual assistant arrangement we've seen: the practice hired well, then handed over a login and hoped. The assistant guessed at undocumented workflows, the guesses were sometimes wrong, the team lost confidence, and everyone concluded remote staffing doesn't work — when what didn't work was a thirty-day period nobody designed. So design it. Here's the week-by-week plan that produces the opposite outcome.

Before day one: the setup week

Three preparations, done before the assistant's first shift, determine half the outcome. Access, properly built: individual scoped credentials in your PM system, phone platform, and communication channel, with MFA enabled — the compliance stack from our HIPAA and security guides, executed once and correctly. The workflow brain-dump: your team writes down, imperfectly, how the first assigned workflow actually runs — scheduling rules, confirmation scripts, the exceptions everyone "just knows." Imperfect is fine; the document improves in week one. The introduction: tell your in-office team what the assistant will own, what stays in the building, and why — relief lands better when it isn't a surprise, and turf anxiety dies fastest in daylight.

Week one: shadow and document

The assistant's first week is absorption, not production. They shadow your workflows by screen share, listen to real calls, read the workflow document against reality, and — this is the productive twist — rewrite it as they learn: every gap they hit becomes a documented answer. By Friday, your practice owns something it likely never had — accurate written procedures — authored by the person who most needed them. Daily fifteen-minute check-ins start now and don't stop this month.

Week two: supervised production

The assistant runs the first workflow — confirmations, say, or verification — with same-day review. Your office manager checks outputs each afternoon: the bookings made, the benefit notes entered, the calls handled. Corrections are specific and immediate ("record IOP time in the note" beats "be more careful"), and they shrink fast; a competent assistant's error rate in week two falls visibly by Thursday. Resist the urge to reclaim tasks after the first mistake — the mistake is the training working.

Weeks three and four: ownership and the second workflow

Week three, the first workflow transfers fully: the assistant owns it, your team stops backstopping it, and review shifts from every output to daily spot-checks. If the workflow runs clean, week four adds the second — same pattern, faster ramp, because the assistant now knows your practice's grammar. If it doesn't run clean, week four is diagnosis instead: is the gap training (fixable), documentation (fixable), or fit (fixable too — that's what the provider's replacement commitment is for, per our vetting guide). Naming which gap honestly, at day thirty rather than day ninety, is the whole value of a structured trial.

The daily check-in, defended

Every element above is negotiable except this one. Fifteen minutes, same time daily, between the assistant and whoever manages them: yesterday's exceptions, today's priorities, any question that would otherwise compound in silence. Practices skip it "once things settle," and the settling is precisely what the check-in produces — tone drift, template misuse, and scheduling-rule confusion all die in that meeting or metastasize outside it. Keep it through day thirty minimum; most practices keep a trimmed version forever because it's the cheapest management they've ever bought.

Day thirty: the honest review

Close the month with the numbers the role was hired to move — answer rate, confirmations completed, verifications done, recall bookings — against the pre-hire baseline you recorded (you recorded it, yes?). Then the qualitative pass: does the team trust the work? Does the assistant have what they need? Expand, adjust, or address — explicitly, on the calendar, not by drift. Onboarding done this way takes one month of deliberate attention and returns a team member who runs their corner of the practice for years. The alternative takes the same month and returns a cautionary tale. Same cost. Choose the design.
